Please use the link below to request an ACRE Certificate or Certificate of Completion for a mini-series. Upon receipt, the Sherlock Center will verify that you have met the requirements of the requested certificate. Requirements include attendance at all required sessions, satisfactory completion of related fieldwork assignments, and a signed ethics statement. You will have an opportunity to electronically sign the "Ethical Guidelines for Professionals in Supported Employment" statement within the request form. We recommend that you read the statement in advance.
NOTE: Names will appear on certificates as entered in the request form and will be mailed to the address provided. Upon submitting the request form you will receive an email confirmation. Please review the information and contact the Sherlock Center with any questions or corrections. You will be contacted by the Sherlock Center if for some reason we are unable to issue the requested certificate or need additional information.
